Immunization in Municipality, Lahan

Immunization of children with target antigen and reaching every child is growing challenge in every municipality. There is an urgent need for coordination and cooperation between municipality and public health authority at local level as well as at ministerial level. Recent findings (research as well as our observations) suggest that rapid urbanization and intra / inter country immigration is a global trend and this has direct/indirect effects on health of children and mothers at every level.

Bird Flu in Banke reports Nepalnews.com

Nepalnews, com has reported Aian Influenza (AI) in Banke (Terai) district in Mid Western Developmental region of Nepal. It is not surprising that AI is detected in Banke due to the porous Indo - Nepal boder, poor border management and weak surveillance system in Nepal. Doubt remains, whether we have been able to detect most of the suspected AI outbreaks?. Please read below published in Nepalnews.com, "Bird flu, the Avian Influenza H5N1 virus, has been detected in various four places of Banke district.District Livestock Services Office, Banke confirmed the prevalence of bird flu in the district on Tuesday after it received a report from the central laboratory in Kathmandu. It had sent samples of poultry products after over 150 chickens and ducks died in a short span in Binauna, Kohalpur, Phattepur and Nepalgunj. Authorities have started taking special measures to control the virus from spreading. They have declared Chacharpharka of Binauna – 4 as the ‘hotspot” and decided to kill and desterilise poultry and poultry products in and around the area. An emergency meeting of chiefs of various offices in the district Tuesday decided to take the special measures at Chacharpharka and 500 metres around it. Although, bird flu was found in Kohalpur, Phattepur and Nepalgunj municipality, authorities have said necessary action have already been taken in those places."

Emerging infectious diseases (In short, EID)

"Emerging infectious diseases" refers to diseases of infectious origin whose incidence in humans has either increased within the past two decades or threatens to increase in the near future.

Strategies that need to be implemented for Coping with EIDs in Nepal.

Surveillance: Detect, promptly investigate, and monitor emerging pathogens, the disease they cause, and the factors influencing their emergence

Applied research: Integrate laboratory science and epidemiology to optimize public health practices.

Prevention and control: Enhance communication of public health information about emerging diseases and ensure rapid implementation of preventable strategies.

Infrastructure: Strengthen village, district and regional level public health infrastructure to support surveillance and implement prevention and control measures.

Outbreak news in "Nepalese Media"

This is a common news headline in Nepalese daily newspaper like "Unidentified disease claims dozen lives in Bhaktapur". The question is what does the word "unidentified" means to the media and general mass. Referring to The Oxford dictionary, it means "nameless, anonymous, faceless, unnamed, unknown, undisclosed, unrevealed, mysterious"

Nepalnews.com on Feb 10, 2010  reported, "At least 11 persons have died in the past one month of an illness with similar symptoms in a Tamang village in Chagu Narayan VDC, Bhaktapur located in the north-east of the Kathmandu valley". Also reports that "panic has gripped the villagers after young people started dying hours after showing symptoms of illness. Everyone who died in the past month showed similar symptoms before death. They complained of headache, temperature and dizziness and oozed out fizz from their mouth and died within hours. The villagers have neither been taking the patients to hospital nor making any efforts for medical treatment. Instead, they resorted to traditional healing including employing traditional healers like Dhamis and Jhankris. VDC secretary of Chagu Narayan informed the public health office of the illness. None of the health professionals have reached the village as of Tuesday. Officials have said, doctors are going to the hospital on Wednesday. Those who died in the past month due to the disease include Rakesh Tamang, 27, Kumar Tamang, 21, Risimaya Tamang, 29, Bila Tamang, 32, and Poornimaya Tamang, 42."

Note:

There are few serious questions that stands out and has to be debated among health policy makers, senior health adminstrators, civil society, local leaders, teachers and other stakeholders.
  1. Why is there such carrelessness on the part of district public health office in responding to this outbreak?
  2. Could it be that the negligence is related to discrimantory attitude on the part of responsible health personnels?

Spot diagnosis

Can you imagine? The white paste applied over the weeping lesion is PARACETAMOL!!!! How come this so? The simple being that there was no health personnel in the sub health post (SHP) and it was by an attendent. However, the disease was identified as scabies and treated with anti-scabies drug and completely cured.

Rubella (German Measles) Outbreak in Himalayan district of Nepal

Picture taken by Anuj Bhattachan (red rash in the face)

There are increasing reports of rubella outbreaks in Nepal, detected through vaccine preventable diseases (VPDs) surveillance network in Nepal. This photo of a child was taken during an investigation of a "suspected measles like illness" outbreak in Mustang, a remote and extremely "hard to reach" Himalayan district in Western part of Nepal.

Follow up note:

Government of Nepal has introduced rubella vaccine through mass vaccination along with measles vaccine country wide. Now, the rubella vaccine is integrated to EPI schedule given at 9 months with measles vaccine.
